All students participating in sports need to complete and return the following forms prior to practice or try outs:
1. Read/Watch steps A-C BEFORE signing Athletic Team Permission Form
A) Concussion Parent Consent Information
B) Concussion – Educational Videos
C) Exertional Heat Illness Program & Annual Review
It is required that parents and student athletes view the educational videos on concussion and Exertional Heat Illness before signing the “Student and Parent Concussion Informed Consent Form”.
3. Sports Health Form
All students participating in sports need an updated health form on record. Each grade has a different requirement. Please click on your child’s grade for the required form or forms.
6th Grade Student – Instead of the “Sports Health Form”, students in grade six are required to have the Connecticut Health Assessment Record (the blue form) on file with the health office. This form can be obtained from the health office, the main office, as well as your physician’s office.
7th Grade Student (Parent Permission Form needed only)
